To the claimant (horse owner):

  1. This free basal ACTH test voucher code is for the laboratory fees for a blood basal ACTH test, performed at Farmlab Diagnostics Ltd, Emlagh Lodge, Elphin, Co. Roscommon or such other laboratory selected by Boehringer Ingelheim Animal Health Ltd from time to time.
  2. This ACTH test voucher code can only be used at your veterinary practice.
  3. This ACTH voucher code is open only to horse owners in Ireland.
  4. Only one claim may be made per horse in a calendar year.
  5. This voucher code will only be accepted if it is genuine and has not been redeemed.
  6. By accepting this offer, you acknowledge anonymised use of your horse’s data and the use of your horse’s results in research projects and studies sponsored or supported by Boehringer Ingelheim Animal Health UK Ltd.
  7. Boehringer Ingelheim Animal Health UK Ltd may only use your personal data for the purposes listed above and in accordance with the terms of this offer and their privacy policy, and will only pass the personal data to selected third party suppliers of Boehringer Ingelheim Animal Health UK Ltd involved in providing services to you on their behalf.

To the Veterinary Practice:

  1. Boehringer Ingelheim Animal Health UK Ltd will pay the separately agreed laboratory fees for a blood ACTH test when this voucher code is submitted to Farmlab Diagnostics Ltd, Emlagh Lodge, Elphin, Co. Roscommon (or such other laboratory nominated by Boehringer Ingelheim Animal Health UK Ltd from time to time) with the blood sample being tested. This is subject to the completion of ALL the required data fields and compliance with the terms and conditions of the promotion including obtaining necessary consents from horse owners to pass their data to Boehringer Ingelheim Animal Health UK Ltd and explaining to them the terms of the promotion.
  2. Please write this voucher code onto the ACTH sample submission form when sending your blood sample to Farmlab Diagnostics Ltd (or such other laboratory nominated by Boehringer Ingelheim Animal Health UK Ltd from time to time).
  3. Payment of the laboratory fee for the ACTH blood test will be made to Farmlab Diagnostics (or such other laboratory nominated by Boehringer Ingelheim Animal Health UK Ltd from time to time) directly by Boehringer Ingelheim Animal Health UK Ltd.
  4. No other arrangement will be entered into and it cannot be set off or used as part payment of any other sum due to Boehringer Ingelheim Animal Health UK Ltd.
  5. The voucher code must be acceptable to Farmlab Diagnostics (or nominated Boehringer Ingelheim Animal Health UK Ltd laboratory) i.e. be legible and comply with the terms of the promotion and used within the Qualifying Period stated.
  6. If a veterinary surgeon or practice has completed documentation or downloaded this voucher code on behalf of an owner, they must advise the owner of the terms of the promotion. The vet must also ensure they have all necessary consents to pass personal data of the owner to Boehringer Ingelheim Animal Health UK Ltd. The vet should advise owners that the test fees are being paid for by the Talk About Laminitis initiative, which is operated by Boehringer Ingelheim Animal Health UK Ltd.
  7. To the extent permitted by law, all contractual or tortious liability (of any kind) is excluded except where negligence causes injury or death.
  8. We reserve the right at any time to update the terms and conditions of this voucher without providing notice to you.
  9. These terms and conditions are governed by English Law.
